Elderly and Disabled Waiver Program (EDWP)

The Elderly & Disabled Waiver Program (EDWP) in Georgia is a State of Georgia Medicaid waiver program administered through either the Community Care Services Program (CCSP) or the Services Options Using Resources in Community Environments (SOURCE). This elderly waiver program is designed to support eligible individuals in receiving care outside of institutional settings.

Hope Private-In Home Care, LLC (Hope Private Care) is certified by the Georgia Department of Community Health to provide these in-home care services to elderly adults age 65 and older throughout Metro Atlanta and the surrounding counties (see a listing of Service Areas covered). We provide Elderly & Disabled Waiver Program (EDWP) assistance to help qualifying individuals remain safely in their homes.

Our Nurses and Caregivers work closely with State-appointed Case Managers to provide all approved in-home care services once you or your loved one is admitted into the program.

Community Care Services Program Overview

The Community Care Services Program (CCSP) is a Medicaid home- and community-based community care program that provides social, health, and support services to individuals age 65 and older as an alternative to placement in a nursing home.

The State has outlined the following services:

• Adult Day Health

• Alternative Living Services

• Emergency Response Services

• Home-delivered Meals

• Out-of-Home Respite Care

• Personal Support Services – In-home services such as personal care, meal preparation, light housekeeping, shopping, and in-home respite services. Based upon eligibility, Structured Family Caregiver provides support to family caregivers who live with and assist an elderly and/or disabled waiver participant requiring significant assistance with activities of daily living.

Service Options Using Resources in a Community Environment (SOURCE) Program Overview

SOURCE operates under authority of the Georgia Elderly and Disabled Waiver Program (EDWP) and provides Home and Community-Based Services to elderly and physically disabled individuals who meet the Intermediate Nursing Home Level of Care.

Available services, as outlined by the State, include:

• Adult Day Health

• Emergency Response Services

• Home-delivered Services

• Personal Support Services – In addition to core services, SOURCE also offers personal support services, assisted living services, extended home health, home-delivered meals, personal care, meal preparation, laundry, bathing, light housekeeping, errands, and in-home respite care.
