We are with you throughout the establishment of your care plan.

Care management is an important process in administering a successful senior care plan. This role provides the Care Plan Manager (a Registered Nurse) with a clear point of reference for changes and development of the care plan. This is a collaborative effort, as Care Managers work closely with caregivers to manage the medical, social, and mental health conditions of our clients more effectively through professional health care management.
 
All live-in and hourly care clients are supported by one of our home care coordinators, who visits each client regularly. To ensure consistent, high-quality, and effective care, our home care coordinator manages and administers medications, orders refills, provides physicians and family members with timely updates, and oversees situations that may arise in the client’s home. In many cases, the coordinator serves as the “eyes and ears” for family members who may live far away from their loved one, supporting continuity within the community care senior health plan model.

  • Identifies client needs
  • Develops a plan to appropriately address the needs and preferences of the client
  • Provides education for family members regarding the client’s condition
  • Evaluates living conditions
  • Identifies appropriate community resources
  • Administers high-tech monitoring programs upon request
  • Writes monthly and quarterly client assessments
